KenyaEMR is very good at what it was built for. A hospital management system is built for something else. Here is how to tell which problem you have, and why many facilities run both.

If you run a Kenyan facility with an HIV comprehensive care clinic, you probably already use **KenyaEMR**. It is the Ministry of Health's open-source electronic medical record, built on OpenMRS and deployed with the support of implementing partners in a very large number of facilities. When someone suggests a hospital management system, the reasonable first question is: why would we need another system?

This is our honest answer, written by a vendor, so weigh it accordingly.

## What KenyaEMR is built for

KenyaEMR grew out of HIV care, and it is very good at it:

- **Longitudinal HIV care**: enrolment, ART regimens, viral load, adherence, appointments.
- **Programme reporting**, including the HIV indicators the Ministry and its partners require, such as MOH 731.
- **National alignment**: it is the Ministry's own system, maintained with partners, and integrated with national programme infrastructure.
- **No licence fee.**

If your problem is running an HIV programme well and reporting it correctly, KenyaEMR is the right tool, and we would not advise replacing it.

## What a hospital management system is built for

A hospital management system solves a different problem: running the whole facility as an operation, including the parts that generate revenue and cost.

- **Registration and queues** for every patient, not only programme clients.
- **Billing**: cash, M-Pesa, SHA and private insurers, deposits, split payments, KRA eTIMS invoices.
- **Insurance claims** built from the clinical record.
- **Pharmacy and stores**: stock, expiry, reorder levels, controlled drugs register.
- **Laboratory, radiology, wards, theatre and maternity.**
- **Facility management**: staff, roles, audit trail, reports across departments.

A facility that is only an HIV clinic does not need most of that. A level 3, 4 or 5 facility whose HIV clinic is one department among twenty does.

## The honest comparison

- **HIV care and MOH 731:** KenyaEMR is built for it. Most hospital systems are not, and AfyaConnect does not attempt it.
- **Billing, M-Pesa and eTIMS:** not KenyaEMR's purpose; core to a hospital system.
- **SHA and private insurance claims:** not KenyaEMR's purpose; core to a hospital system.
- **Pharmacy stock, wards, theatre and maternity:** outside KenyaEMR's focus; core to a hospital system.
- **Licence cost:** KenyaEMR has none; a hospital system is usually a subscription.

## Running both

Many facilities run KenyaEMR in the comprehensive care clinic and a hospital management system for everything else. That is a sensible arrangement, not a compromise, provided you are clear about which system is the source of truth for what.

- **HIV programme data lives in KenyaEMR** and is reported from there.
- **Billing, stock, claims and the rest of the facility live in the HMS.**
- **Don't double-report.** If HIV indicators are reported from KenyaEMR, the HMS should leave those lines blank on shared returns, not report them as zero.

As the Digital Health Act's exchange matures, systems that speak FHIR will be able to share records rather than ask staff to retype them. See our guide to [the Digital Health Act and your hospital system](/resources/digital-health-act-kenya-hmis-compliance).

## Where AfyaConnect fits

AfyaConnect is a hospital management system. It does not try to replace KenyaEMR for HIV care, and it does not report the HIV lines that belong to MOH 731: on shared returns such as MOH 711 those lines are marked as not collected. It runs the rest of the facility: registration, billing, claims, pharmacy, laboratory, wards, maternity and the monthly MOH returns.

The team that builds and supports AfyaConnect writes these guides. Everything here is written against software we actually ship and support in live Kenyan facilities — the SHA claims flow, the PPB controlled substances register and the eTIMS invoicing described in these guides are modules we maintain, not features we read about. Where regulation is still moving, we say so rather than guessing.

- Supports 40+ healthcare facilities across Kenya and East Africa
- Builds and maintains the SHA/NHIF claims, PPB register and KRA eTIMS modules described in these guides
- Works to the Kenya Data Protection Act 2019 and Digital Health Act 2023 requirements for patient data
