One record from the first antenatal visit through labour, birth, the newborn unit and postnatal care, with immunisation and family planning registers that fill MOH 711 for you.

## What's included

- Antenatal clinic with a running ANC profile: visits, tests, risk factors and the expected date of delivery
- Labour ward with a digital partograph
- Births register with APGAR scores, newborn admission and a newborn unit
- Postnatal care for mother and baby
- Immunisation register with defaulter tracing
- Family planning register (MOH 512)
- MOH 711 compiled from the ANC, immunisation, family planning and growth registers, ready for KHIS

Maternal and child health is where a facility's record-keeping is tested hardest. A woman may be seen eight times before delivery, by different staff, and the one thing that matters on the day of labour is whether the risk recorded at the second visit is visible at two in the morning.

AfyaConnect keeps one record from the first antenatal visit to the child's last scheduled immunisation.

## Antenatal care

Each ANC visit adds to a running profile: gestation, blood pressure, tests, risk factors and the expected date of delivery. A clinician opening the record sees the pregnancy at a glance, not a stack of visit notes to read in order. High-risk factors carry forward, so a history recorded at the first visit is in front of the midwife at delivery.

## Labour and delivery

The labour ward uses a digital partograph. Observations are plotted as they are recorded and the alert and action lines are drawn for everyone on the ward to see, which is the point of a partograph in the first place.

Births are recorded with APGAR scores. The newborn is registered as a patient in their own right, linked to the mother, and can be admitted straight to the newborn unit.

## Postnatal care

Postnatal visits are recorded for mother and baby together, because they are usually seen together, while each keeps their own record.

## Child health

- **Immunisation register** with the schedule per child and a defaulter list for follow-up.
- **Growth monitoring** recorded at each contact.
- **Family planning register** (MOH 512).

## Reporting without tallying

The monthly MOH 711 is compiled from those registers. The work that used to take a nurse in charge an evening at month end becomes a report you check and submit. Lines the facility does not collect are marked as not collected, never quietly reported as zero. Our [guide to KHIS monthly reporting](/resources/khis-moh-705-711-monthly-reporting-guide) explains how the returns fit together.

## Who it is for

Health centres and maternity homes that are mainly MCH, and hospitals whose maternity wing is one department among many. Maternity runs on the same patient record as the rest of the [hospital management system](/solutions/hospital-management-system-kenya), so a mother referred to theatre for a caesarean section arrives with her history. [Register your facility](/hospital-signup) to see it with your own workflow.

## Frequently asked questions

### Does the partograph replace the paper chart?

It can. Cervical dilatation, foetal heart rate, contractions and maternal observations are plotted as they are recorded, so the alert and action lines are visible to anyone on the ward rather than only to the midwife holding the paper. Many facilities run both for a short period while staff build confidence, then retire the paper.

### How is MOH 711 produced?

From the registers the clinic already keeps: antenatal visits, immunisations, family planning visits, growth monitoring and TB screening. Each service is counted where it is recorded, so the monthly return is a report rather than an evening of tallying. Lines the facility does not collect, such as the HIV lines reported through other systems, are left as not collected rather than sent as zero.

### Can we trace immunisation defaulters?

Yes. The immunisation register knows which antigen each child is due for and when, and lists the children who have missed a dose so the clinic or a community health promoter can follow up.

### Is the baby a separate patient?

Yes. A newborn is registered as their own patient at birth, linked to the mother, so the newborn unit, immunisations and growth monitoring follow the child while the delivery stays on the mother's record.
